英文公證書的格式比較規范，一般來說，公證地名寫在左上方，須全用大寫字母，ss.（或SS.)寫在地名中間。這個縮略語的意義不詳，有人認為它代表拉丁字cilicet，表示to wit或namely（“即”），也有人認為它有其他來源及含義。公證書正文應和地名相隔三行。
